Incident Reports(사고 보고서)
An incident report is your record of a situation that has occurred within your establishment and how it was handled. It is important to complete an incident report while details are still fresh in everyone's mind as it may be used as evidence if the situation becomes a legal matter.
사고보고서는 당신 영업장 안에서 일어난 상황과 그것이 어떻게 처리 됐는가 하는 것의 기록이다. 상황이 법적인 문제가 될 경우를 생각해 세부사항들이 아직 모든사람들 기억에 생생하게 남아있는 동안 사고기록을 마쳐놓는 것이 중요하다.

An Incident Report should be completed when:
사고보고서가 작성돼야 할때
An intoxicated guest is refused service
취한 사람이 술제공을 거절당했을 때
A minor presents false ID
미성년자가 가짜 ID를 제출했을때
A guest becomes violent or a fight breaks out
손님이 폭력적으로 되거나 싸움이 발생했을때
Any time a guest is asked by staff to leave
손님이 스태프로부터 떠나달라고 요청 받았을때
You arrange transportation for an intoxicated guest
술취한 손님을 위해 교통편을 조치해 주었을때
A liquor inspector visits your establishment
주류 인스펙터가 영업장을 방문했을때
A guest has an accident or becomes ill
손님이 사고를 당했거나 아프게 됐을때
Police are called

의복 종류나 스타일
Attitude

Condition of guest (note signs of intoxication if applicable)
손님의 상태 (가능하면 취했다는 징후를 기록해 놓는다)
Details about other guests in the party
파티에 있던 다른 손님의 세부사항
Witnesses (their name and address if possible)
증인 (가능하면 그들 이름과 주소)
Information on whether the guest was driving
해당손님이 운전을 했었는지에 대한 정보
Photocopy of guest's sales slip or guest check
손님이 주문한 명세서와 영수증 복사본
Steps taken to ensure safe trip home (i.e. alternative transportation)
